You may refill a prescription by fax or mail. Please follow these steps:

Refill Form.
Complete fully and sign the above forms.
Send the form via fax or mail.
Please note: if there is a change in your prescription we will require a new prescription from your physician outlining the changes. In this case you will need to send in the original written prescription.
